I think he means that the inside structure is missing. The middle tube is empty.

From all I can say all sides are rounded. But don't have the part either...

It looks and feels the same as the normal smooth axle holder, two rounded sides and two flat sides. The top and bottom sides are rounded, the sides are flat (with the current orientation of the part)

I just looked at BIs part, all sides are rounded there. But it wouldn't be the first time that a BIs mesh is inaccurate...

Let's keep things simple please! It's functionally OK. Quote of Eurobricks: "Notice that one can only insert an axle by 1L (but a bar can go all the way through)"

IMHO , the blocker serves it's purpose inb the current shapeso IMO it is good that way
Axle needs to be stopped -> check
Bar needs to go through -> check

We really need to consider the tapered and shortened Axle ends now... to keep them from collinding in parts like this.
