Not completely sure I understand what you mean.
The only area ever printed on this piece are the eyes. Yes, the mouth actually has the minifig torso's neck connector partially exposed due to open mouth. Though this has never influenced the neck mark printing in any way. I doubt they could use that for some effect, even if they wanted.
Funnily the backside of this head seems rather identical to a standard head (or at least heavily based on one).
